Humans are simply not good at passing off 80-plus percent of a task and then staying alert to monitor what’s going on, which is what Autopilot demands. Since Level 2 systems offer no failover capability and need a human to be ready to take over at any moment, if you’re not paying constant attention, the wreck we see here is precisely the kind of worst-case nightmare that can happen.
The autopilot system turned off. The person in the driver's seat too over 100% of the car's operation. I fail to see at all how this is "level 2" autonomous control's fault. Because an automatic braking system didn't kick in at 130mph? Isn't this like blaming an unused parachute for someone dying in a plane crash?
Depends on whether the parachute was actually unused or it was but the user missed the giant gaping hole at the top of it.
If lane keep assist was on and working, but Automatic Forward Collision braking wasn't, it's possible that the driver thought autopilot was still enabled. After all, the thing was still steering itself. This does not excuse them accelerating but it makes the single bong warning you autopilot was disabled was a lot easier to miss. Also, quite why would autopilot disengaging would also deactivate auto emergency braking is beyond me.

If I may continue to pry, what exactly does the car do when autopilot is disengaged if it's not a single bong? Furthermore, is there anything on the UI that would tell you if auto emergency brakes are switched off? I feel that should be at least an icon.
