Use just the video ID, ie.

http://www.youtube.com/v/p64yUG3VAVE

The tag is p64yUG3VAVE. Enclose that in the Youtube tags:

[youtube]p64yUG3VAVE[/youtube]

Gives you:
[youtube]p64yUG3VAVE[/youtube]

Or, to add a description:
[youtube=p64yUG3VAVE]Blah Blah[/youtube]

That's using the http://www.youtube.com/watch?v= type of link

Yep, use the ID or the URL you see in your browser, not the URL they give you for the embed. ;)
